About the Lifetime Care Foundation

logo_lifetimecare.pngTHE ROSE AND MAURICE HALPERN LIFETIME CARE FOUNDATION AT OHEL ensures that a person with mental illness or developmental disabilities will receive high quality care and services beyond the lifetime of his or her parents with guardianship, advocacy, case management, and supplemental needs trusts.

PSYCHIATRIC CARE MANAGEMENT

For the psychiatric population we provide assistance with:

1. Housing Options: We assess the individual's housing needs and make referrals to appropriate placement into residential or supportive apartment programs.

2. Case Management Services: We provide regularly scheduled visits to monitor the individual's medication regimen, maintain the apartment, make sure the bills get paid on time, ensure that scheduled appointments are kept and help keep him or her stable.

3. Referral and Advocacy: We assist the individual to ensure that he/she receives appropriate clinical supervision. We refer the individual to resources that can be helpful to them and provide advocacy to help the individual to achieve their goals.

4. Day Structure: We provide assistance with creating a day structure through a day program, day treatment program, or supported employment.

5. Family Work: We liaison between the family and treatment team to ensure the individual is receiving the highest level of care.

6. Government Services: We provide assistance with eligibility for Community Medicaid, Medicare, Food Stamps, SSI and SSA.

GERIATRIC CARE MANAGEMENT

For the Elderly population we provide assistance with:

1. Housing Options: We assess the individual's housing needs and offer consultations regarding appropriate placement into adult homes, assisted living facilities, or nursing homes.

2. Government Services: We provide assistance with eligibility for Medicaid, M11Q's, Medicare, Food Stamps, SSI and SSA.

3. Referrals: We refer people to elder care attorneys, medical doctors, mental health professionals, home care services, cleaning agencies, meals on wheels, transportation and handypersons.

4. Family Work: We liaison between family and care providers to ensure that the individual receives the highest level of care. We make frequent home visits to establish a trusting relationship with our client and function as an additional pair of eyes for family members. And we set goals with the client and family to ensure the highest level of functioning.

INCOME ONLY TRUSTS FOR SENIOR CITZENS

Pooled Trusts:

Attention Senior Citizens and Concerned Family Members:

Are you or your loved one in need of or currently utilizing home-care services, due to disability? Perhaps you are unable to begin or will need to discontinue such services due to their prohibitive costs. If this situation describes you, we have good news: Disabled seniors can qualify for Medicaid, even with income over the Medicaid limits. To date, disabled seniors have been required to either ‘spend down' their surplus income on Medicaid-approved expenses, or refund this surplus income to Medicaid. However, recent rulings allowing the deposit of surplus monthly income into Supplemental Needs Trusts are making a significant difference for thousands of seniors in New York State. The Lifetime Care Foundation for the Jewish Disabled has a Community Pooled Trust operating as a Supplemental Needs Trust, which can allow you, upon advice of an attorney, to make monthly deposits of your excess income into the trust, and therefore become and remain Medicaid eligible. In turn, this money is now available to be disbursed for your benefit for reasonable living expenses. These trusts can enable seniors to use their excess income to pay for rent, utilities and medical services not covered by Medicaid. The Lifetime Care Foundation does not offer legal advice, therefore it is important to discuss this with your attorney and/or financial advisor in order to help maximize the benefits and services you are entitled to receive under current laws and regulations.

SUPPLEMENTAL NEEDS TRUST

A Supplemental Needs Trust is a legal document created for persons with physical, mental or developmental disabilities to have assets held in Trust for his or her benefit. Funds in a trust are not considered assets for purposes of certain governmental benefits. Parents and family of persons with disabilities who are doing estate planning, as well as persons receiving payment from lawsuits or other income who have disabilities, can use a Supplemental Needs Trust to hold assets for their use without endangering eligibility for Medicaid or SSI. The Lifetime Care Foundation manages a Pooled Community Trust which functions as a Supplemental Needs Trust

NEW LEASE ON LIFE PROGRAM

New Lease on Life Program

When Someone You Love Suffers From Chronic Mental Illness or a Developmental Disability, Where Can You Find A Place To Live? The Lifetime Care Foundation offers intensive case management services through our New Lease on Life Program. We provide daily visits to monitor the individual's medication regimen, maintain the apartment, make sure the bills get paid on time, ensure that scheduled appointments are kept and help keep him or her stable.

The Lifetime Care Foundation New Lease on Life Program

Offers Immediate help in Placement Day Structure Rehabilitation.

Through this program, your loved one will gain:

  • Dignity and Independence in his or her own apartment
  • Daily visits from a warm, caring professional who will assist in maintaining the apartment and help keep him or her stable
  • Optimism and hope that a person CAN recover and maximize their functioning, from an organization that has a proven track record of over two decades
  • Find a quick solution to housing problems and avoid a long waiting list.

Applicants from outside New York State are Welcome! The Lifetime Care Foundation New Lease on Life Program is not funded by government programs and requires financial contributions by participants. The program will work with you to maximize your entitlements and benefits from government programs and help you qualify for government-funded housing options as they become available.
