Geriatric Care Management

OHEL Offers A Breadth of Highly Specialized Eldercare Support Services Under One Roof

Optimizing Quality of Life and Providing Peace of Mind
For over 43 years, OHEL has been meeting the diverse needs of the eldercare community. Now, with the baby boom generation coming of age, there is an ever increasing need to provide effective and efficient services that meet the needs of both seniors themselves and the caregivers in the family.

OHEL Caregiver Help Line and Support Program
Many people caring for a loved one don’t think of themselves as caregivers, and often underestimate the stress and impact it can have on their lives. Supportive family members can quickly find themselves in alarmingly stressful situations trying to manage a crisis in an unfamiliar environment and even the best families can become mired in conflict. The circumstances of each family are unique and OHEL provides specific eldercare solutions based on both the needs of the senior and the dynamics of the family.

Case Management
OHEL works to effectively manage and ensure the best interests of each client. OHEL helps navigate the maze of options and services and this includes one on one comprehensive and personalized case management on a fee basis, client advocacy, access to entitlement programs, and educational, social, and recreational support services.

Supported Housing for Seniors
OHEL’s geriatric specialists are driven to ensure as independent a life for seniors as possible. A new OHEL senior housing project ensures seniors can live independently, receive weekly visitations from OHEL social workers and other on-call services, in a modern apartment building that includes a gym, hair salon, and private beach, in a beautiful neighborhood.

Mobile Home Visits
Many seniors find themselves housebound. OHEL is here to help, in the home. OHEL offers supportive home visitation services by mental health professionals for the elderly who have difficulty leaving their place of residence.

Pooled Trusts
A frequent challenge amongst seniors is qualifying for Medicaid services. OHEL geriatric financial experts help disabled seniors qualify for Medicaid and home-care services, even with income that is over the Medicaid limits.

Individual and Group Counseling
Seniors can especially face an extraordinary number of emotional and personal challenges. OHEL Geriatric counselors are here to help. Our experienced mental health professionals who speak English, Hebrew, Yiddish and Russian are sensitive to the different cultural needs of the community. Services are available at OHEL’s Regional Family Centers in Brooklyn, Far Rockaway and Teaneck, New Jersey.
