Help Navigating Systems to Get Medical and Mental Health Care

Hub@Home Case Management staff refer seniors to medical and mental health services to help seniors cope with mental health issues. We provide ongoing guidance on aging-related concerns, healthcare options, insurance, and available community resources. Our experts can help seniors find critical services such as Food Stamps, SCRIE, Medicaid, Access-a-Ride, and more.
