Managing chronic medical or Mental health issues alone can be overwhelming, but through Health Homes, Ohel’s program for Case management for adults and children,  treatment and lives can get back on track.  Open to any individual who receives their health benefits through Medicaid, this program helps navigate day-to-day challenges, including finding doctors, housing, and accessing education.

Health Homes has helped clients who were living in their cars apply for and obtain affordable housing. Clients who had trouble managing severe diabetes have received help understanding their diagnosis, obtaining medication, and using it correctly. Health Homes has helped clients to take better care of themselves so they can avoid emergency room visits and hospitalizations. “Our job is to look at each client as a whole person. We ask, ‘What are your needs? What are your goals that aren’t being met?’ We’ll help you meet and even exceed those needs and goals,” said Lisa Sheinhouse, director of Community Services.
